Take Route 21 South out of Mineral Wells to Wolf Run Road in Rockport. Follow Wolf Run for about 3 � miles to Slate Creek Road. Follow Slate Creek for about 2 � miles. The church and cemetery are next to the road on the right side.

This cemetery is owned and maintained by the Slate Chapel United Methodist Church. The cemetery is no longer active, but the church still holds services.

The church was established in 1878, but many of the burials pre-date the church. Many of the stones have been repaired numerous times and are very worn and hard to read. Please verify the information with other sources.
